Admissions Coordinator Nurse, RN or LPN (FT, Mon-Fri, 9:00a to 5:30p)
The Admissions Coordinator Nurse, reporting to the Director of Access Services, assumes responsibility for coordination of the Hospice of the North Shore & Greater Boston (HNSGB) admissions department, which includes the Referral Center and Admissions Liaisons/Nurses. In this hands-on coordination role, the Admissions Coordinator Nurse ensures consistency in quality of the referral and admission process, ensures compliance with applicable regulations, and serves as a liaison to the community on issues related to referrals and admissions for hospice and palliative care services.
Qualifications: Professional Registered Nurse (RN) or LPN, currently licensed in Massachusetts. Bachelor's degree in nursing or related health care field preferred, with minimum of three years related experience in hospice, oncology, home care or long term care. Previous leadership/supervisory experience preferred. Demonstrated skills in communication and outreach. Ability to guide and develop the skills of professional and non-professional staff and exercise independent judgment. Excellent verbal and written communications skills with individuals and groups. Admissions Liaison RN, Southern Urban Tier (20 hour, benefited position, Wed, Thurs & every other Friday, 12:00p to 8:30p)
Reporting to the Admission Manager, the Admissions/Liaison Nurse is responsible for patient assessments and admissions to HNS, ensuring continuity of care between the admitting process and transition to ongoing care by the hospice interdisciplinary team (IDT). In addition, the Admissions/Liaison Nurse is responsible for educating health care professionals, patients, and families in all settings regarding the benefits/appropriateness of hospice care. The Admissions/Liaison Nurse is responsible for being proactive in identifying new referral sources, increasing referrals from existing sources, and providing educational programs to facilities. The Admissions/Liaison nurse maintains good communication with HNS clinical staff in order to enhance hospice services, and facilitates communication between HNS and referral sources. Will be will be responsible for traveling in our South Tier which will cover anywhere from Wellesley, Waltham, Milton, Needham and Norwood to Quincy, Braintree, Boston, Newton and surrounding towns.
Qualifications: Current MA RN license in good standing. Minimum of three years of varied work experience as a professional nurse, preferably one of those years in an acute setting. The Quality Coordinator works in partnership with the Director of Quality and Performance Improvement to assess and improve all aspects of Home & Hospice Care of Rhode Island's quality metrics for both patient care and operations. As a quality resource, this individual is responsible for coordinating the implementation of identified quality initiatives in departments across the organization.
The Quality Coordinator must have a passion for end of life care, a solid knowledge base grounded in clinical knowledge and regulatory expertise and enthusiasm for driving performance improvement throughout the organization for the benefit of its patients and their families.
On a regular basis, the Quality Coordinator assists in the development and deployment of an organization-wide Quality Assessment / Performance Improvement program by (1) Leading Patient Care Managers, supervisors and other clinical staff across the organization to identify their concerns with care issues and to identify areas for improvement; (2) Independently creating data and tools to capture changes in quality processes; and (3) Utilizing critical judgment to prioritize action steps for quality improvement.

The successful candidate will possess at least five years healthcare experience with in-depth clinical knowledge to include a minimum of two years nursing experience in a Home Care/Hospice setting. Experience with CHAP survey and/or in quality improvement in a health care setting preferred.

This role is responsible for the completion of the patient admission process which includes: review of admission packet with patient and family/caregiver/health care proxy; explain and clarify hospice philosophy; benefits to be expected through HHCRI; how to contract HHCRI, liability for services (if applicable); inform patient of rights and responsibilities; and have patient or health care proxy sign consent forms.

Registered Nurse – Referral Center Part-time, 12 & 16 hours/ week, 5-9pm
As an office-based RN, the Referral Center Nurse ensures a smooth transition for patients being admitted to Home & Hospice Care of Rhode Island. Provides timely, responsive customer focused and customer friendly service to all referral sources, patients, families, staff, students, vendors and visitors. Acts as a liaison for incoming calls to coordinate visits with the Admissions Nurses and Nurse Liaisons.
The Referral Center RN acts a customer service liaison between referral sources, care providers, professional staff, patients and families, insurers, physicians and members of the IDG to ensure the smooth and timely admission of patients. Through phone and internet communications, establishes and maintains a positive relationship with all internal and external customers.
Schedules visits for all inquiry and referral callers within required timeframe. Arranges visits for liaisons and admissions nurses, providing follow up on pending patients and non-admissions. Communicates prospective patient updates to appropriate referral sources.
On a regular basis, communicates with referral sources, DME, pharmacy, and other vendors to process all certifications, orders, and required admission information. Helps resolve referral and admissions scheduling issues and customer complaints through direct communication with the referral sources, admissions team, Help Center & Admissions Manager and other managers.

The Inpatient Admissions Nurse welcomes and admits patients and families to HHCRI care. Provides skilled professional nursing care to the patient and family as prescribed by the physician and in compliance with regulations and accepted standards of practice as established by the State Board of Registered Nursing, accrediting bodies, and the policies of Home & Hospice Care of Rhode Island through a competent, compassionate, timely and responsive admission process.
This role functions as first face-to-face HHCRI contact for patients, families/ caregivers in the Philip Hulitar Inpatient Center. Will provide assessment, consultation, support and education to patients, families and caregivers and others in the setting in which hospice care will be provided.
As the Admissions Nurse, will complete the admission process which includes: review of admission packet with patient and family/caregiver/health care proxy; explain and clarify hospice philosophy; benefits to be expected through HHCRI; inform patient of rights and responsibilities; and have patient or health care proxy sign consent forms. Initial physical assessment of patient will only be completed after the above has been explained and the consents are signed.
Provides professional nursing care by utilizing all elements of nursing process. Coordinates all aspects of patient care on initial assessment; Uses interdisciplinary approach in developing plan of care, in accordance with the organization's policies, in collaboration with patient/caregiver/primary health professional.
Performs initial assessment of patient and family to determine the impact of the terminal diagnosis on the patient's physical, functional, psychosocial and environmental needs and activities of daily living. Initiates and applies appropriate preventive and therapeutic nursing procedures and techniques, maintains communication regarding patient care in collaboration with the primary care providers and associated health professionals. Coordinates patient services with other team members to provide continuity of care. Works in cooperation with the family/caregiver and interdisciplinary team members to meet the identified needs of the patient/caregiver; including psychosocial, spiritual and emotional needs. Documents in Allscripts and verbally reports off to team to which patient is assigned, the outcome of admission in expected time frames to insure continuity of care to patient, caregiver, and designated members of the HHCRI team.
The successful candidate will possess a Bachelor of Science Degree in Nursing preferred; two years' related experience or training; or related equivalent combination of education and experience. Reporting to the Admission Manager, the Admissions/Liaison Nurse is responsible for patient assessments and admissions to HNS, ensuring continuity of care between the admitting process and transition to ongoing care by the hospice interdisciplinary team (IDT). Admissions Liaison RN, Southern Urban Tier (Full time, benefited position)
Full-time, benefited position, 8a-4:30p with minimal occasional on-call requirement
Reporting to the Admission Manager, the Admissions/Liaison Nurse is responsible for patient assessments and admissions to HNS, ensuring continuity of care between the admitting process and transition to ongoing care by the hospice interdisciplinary team (IDT). In addition, the Admissions/Liaison Nurse is responsible for educating health care professionals, patients, and families in all settings regarding the benefits/appropriateness of hospice care. The Admissions/Liaison Nurse is responsible for being proactive in identifying new referral sources, increasing referrals from existing sources, and providing educational programs to facilities. The Admissions/Liaison nurse maintains good communication with HNS clinical staff in order to enhance hospice services, and facilitates communication between HNS and referral sources. Will be will be responsible for traveling in our South Tier which will cover anywhere from Wellesley, Waltham, Milton, Needham and Norwood to Quincy, Braintree, Boston, Newton and surrounding towns.
Qualifications: Current MA RN license in good standing. Minimum of three years of varied work experience as a professional nurse, preferably one of those years in an acute setting. Hospice nursing experience is highly desirable. One year additional work experience as a professional nurse in a homecare, hospital or skilled nursing facility with strong clinical assessment skills related to admission to or change in service. Must obtain and maintain certification for your position as defined in the HNSGB's Operation Policy. Demonstrated understanding of Medicare/Medicaid benefit as it relates to all facilities settings. Registered Nurse Case Manager (1 FT Positions, Homecare, North Shore Area)
Our growth and expanded service has created opportunities for nurses with or without hospice experience. We currently have openings in our Homecare division in which each will be responsible for traveling in the North Shore area. Our nurse to patient ratios make Hospice nursing especially attractive to nurses who work for other hospices. The On-Call Nurse works under the supervision of the Clinical Manager or designee during scheduled on-call hours. Responds promptly from home and with sensitivity to telephone inquiries from patients, family members, physicians, nursing homes and others regarding clinical issues. Utilizes clinical judgment to determine level of patient need, and make home visits as indicated and/or assigned. Schedule includes 7 days on and 7 days off from Wednesday to Wednesday, 4:30p to 8:00a on weekdays and 6:00p to 8:00a during the scheduled weekend, totaling 70 hours biweekly. These are expansion positions as there are already On Call RN's in place. This position will cover the Metro Boston territory for the Wellesley office.

Social Worker-Kaplan Family Hospice House ( Mon, Wed, Fri 5p-9p, 12 hours, PT)
As a member of the interdisciplinary team, the social worker reports to a Clinical Manager, and is responsible for planning and providing supportive services to terminally ill patients and families in the North Shore area from Methuen to Gloucester and towns in-between. The social worker is responsible for identifying patient/family psychosocial needs, and for addressing those needs through direct service and/or consultation with other staff members in accordance with the hospice plan of care. Will carry a continuous case load and will take part in Social Worker/Chaplain On-Call Rotation.
Qualifications: Master's degree in social work or related field preferred. Current Massachusetts social work licensure (LICSW preferred, LCSW accepted with appropriate work experience). Minimum three years counseling experience. Experience counseling terminally ill persons and their families; knowledge of the health care delivery system and community resources.

Kaplan Family Hospice House: Danvers (Sat & Sun, 11p-7:30a, every other weekend)
HomeCare/LTC Team: Wellesley Office (Full Time, M-F, 08a-4:30p with rotating WE and Holidays)
The Hospice House Hospice Aide is a vital member of the Hospice House interdisciplinary team. He/She is to care for patients in the Hospice House setting. The Hospice House Hospice Aide functions under the direct supervision of the Hospice House Manager with additional direction from the primary care nurse responsible for the particular patient.
Qualifications: High School Diploma or equivalent preferred. Certification of successful completion of a CNA or CNA/ Home Health Aide course approved by the Massachusetts Department of Public Health and in accordance with the requirements of the U.S. Department of Health and Human Services. Certification must be in good standing with the nurse aide registry and not have had a lapse in furnishing services within a 24 month period. In lieu of an HHA certificate, successful passing of hospice aide written test and skills assessment.
Per Diem RN, Greater Boston Area (weekday or weekends)
The Per Diem Nurse works under the supervision of the Clinical Manager or designee. Assignments and hours worked are arranged depending on the patient caseload and the pre-arranged availability of the per diem nurse. Responsible for identifying patient/family needs and for providing supportive care in accordance with the attending physician's orders and plan of care and Hospice of the North Shore & Greater Boston's policies and procedures.
Qualifications: Registered Nurse, currently licensed in Massachusetts in good standing. Preferably a Bachelor's degree in nursing. Must obtain and maintain certification for your position as defined in HNSGB's Operation Policy: Clinician Certification. Minimum three years of varied work experience as a professional nurse, and preferably one of the three years in an acute setting. Demonstrated ability to assess and respond to the needs of patients and families in varied settings. Skilled in nursing practice, able to cope with family emotional stress and tolerant of individual lifestyles. Demonstrated understanding of hospice philosophy and principles. Demonstrated ability to be self-directed, flexible and cooperative in fulfilling role obligation and ability to work effectively within an interdisciplinary team. Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and in good working order. Shares in providing 24-hour, seven-day-a-week coverage to patients/families. Work a minimum of one weekend per month and one holiday per year, and assumes responsibility for a proportionate share of on-call coverage, as needed. Obtains relevant clinical information from patient records and from other staff members to ensure necessary familiarity with patient/family situations which require intervention. Communicates pertinent information to primary nurse and/or designated manager. Maintains regular and effective communication with other clinical staff, so that patient activities are coordinated and consistent.
Per Diem Nurse Practitioner (North Shore area)
Under the clinical supervision of the Palliative Care Team Physician, the Palliative Care Nurse Practitioner (PCNP) provides consultation services as requested by area clinicians to address complex pain and symptom management issues for patients experiencing chronic and/or progressive life-limiting illness and their families. The PCNP participates in patient/family/provider education, as well as palliative care research and development opportunities for the Hospice of the North Shore. The PCNP engages in outreach activities to support the mission of Hospice of the North Shore (HNS).
Qualifications: Professional registered nurse, currently licensed in Massachusetts. Certified Adult Nurse Practitioner; certification in advanced practice palliative management preferred. Bachelor's degree in nursing required; Master's degree strongly preferred. Minimum of five (5) years nursing experience preferred. Minimum of two (2) years experience as a nurse practitioner preferred, ideally in an oncology, palliative care or hospice setting.
