Speech Language Pathologist , Per Diem-- 56359
The certified Speech Language Pathologist provides diagnostic and therapy services to an assigned caseload. The therapist is responsible for demonstrating when necessary, skills of communication, to other rehab and supportive personnel which would assist in caring for individual patients. We are looking for someone who can provide evaluations in the home and continue to treat in home.
Requirements:
This is a per diem position, must be very flexible.
Graduate from an accredited Speech Language Pathologist program and holds a current Massachusetts Speech Language Pathologist license, current CPR certification and valid driver's license. Minimum of one-year clinical experience preferred.

- Development of individualized plans of care for patients within 48 hours of admission.
- Provision of direct care, education and support to patients and family members.
- Management of medical regime as prescribed by attending physicians.
Requirements
- Current and valid RN License in MA
- Experience in either hospice, home health, primary care clinic or other health facility
- Strong communication skills.
- Reliable transportation and valid driver's license.

Hospice Clinical Manager
A registered nurse, experienced as a Hospice home care case manager of similar experience. He/she is responsible for the clinical nursing services of the Hospice program and works in conjunction with other personnel, such as billing and medical records, to ensure optimal management and consistency for all aspects of patient care.
Must be a graduate of accredited school of nursing. Must have a current MA or NH license. Must have at least two years of experience as a case manager in community health nursing, preferably in a Hospice setting. Minimum of tow years in a supervisory position. Experience with terminally ill patients and issues of palliative care, preferably in a Hospice setting.
